Fall Activity Registration And Back To School Forms

Dear Parents,

To prepare for the start of the 2022-23 school year, there are a number of forms that we need to collect to ensure the safety and wellbeing of your child while at school and participating in activities.

Fall Activity Registration (Registration period July 26-August 1)

Prior to the first day of Fall tryouts/practice (August 1st), every student athlete must complete the Fall Activity Registration. Marching Band, Flag Corps, and Dance Team also need to complete the Fall Activity Registration.

Back to School Forms (Registration period July 26-August 23)

Prior to the first day of school (August 24th), EVERY student must complete a series of Back to School Forms.

You can find all of these forms in your PowerSchool Parent Portal under the "Forms" tab. To log in, visit the SFD Website or go to desales.powerschool.com. This must be completed through a web browser, the PowerSchool Mobile App will not work!

Have an incoming freshman? We have added your incoming student to the same account that you have used in the past for your current student. There are separate tabs for each child. Please note, you will need to complete forms for EVERY child. New incoming freshman families will receive an email from Mrs. Julie Barber later today with your PowerSchool login information.

If you have forgotten your PowerSchool Parent Portal password, please feel free to contact admissions@desales.co or powerschool@desales.co and we can reset it.
