There are three main sections of the permit. 

First is the Student/Applicant Information.  All sections must be completed by the student.

Section two is the Pledge of Employer.  This section is completed by the employer. Again, ALL sections must be completed by the employer.

And lastly, the Physician's Certificate for Minor Work Permit.  This section must be completed if the student does not have a current physical on file (within the past 12 months) at St Francis DeSales.

Once everything has been completed, please see the Registrar or either the Attendance Secretary or Executive Assistant to the Principal in the Main Office to be processed.  Once the permit is processed via the State Of Ohio’s website, the work permit will be printed and given to the student to submit to their new employer.
